For support coordinators

For Support Coordinators: Clear Communication, Thoughtful Intake

Westmore values responsive, practical collaboration with Support Coordinators, families, and other members of the participant’s team. Referrals are reviewed for service alignment, support needs, scheduling, transportation feasibility, staffing, and program fit. Questions about medical, behavioral, mobility, dietary, or accessibility needs can be discussed directly with Westmore leadership before intake is finalized.
